The Weekly NewsDigest and Its Remarkable Editor

Even close readers of the Weekly NewsDigest may be surprised to know that we are closing in on 18 years of weekly updates. Much has changed over these years – the HIV prevention news itself, the media world, and the technologies that help us compile and edit each issue. But there has always been one constant – the remarkable Polly Harrison!

Polly the advocate, scientist, policy wonk and editor is first and foremost an intrepid guide to all of us who have learned from her and been inspired by her – and that includes just about everyone working in HIV prevention research.

Every Friday for over 18 years, thousands of people receive what looks like a simple email digest of news covering HIV prevention research and relevant science in peer-reviewed journals. But it has always been so much more than that, thanks to Polly!

The Digest was Polly’s brainchild at the Alliance for Microbicide Development, which she founded as a catalyst for the microbicide field. The Alliance and AVAC were always close collaborators and share an unyielding commitment to raise awareness and provide a neutral, objective basis for decision-making and evidence-based advocacy. So when Polly “retired” the Alliance 8 years ago, AVAC was incredibly fortunate to make sure that she herself didn’t retire.

As our Senior Advisor on so many things over the years, Polly guided the Digest at its new home – ensuring your weekly summary arrived on schedule and mentoring all of us at AVAC – and made us all marvel how anyone of any age could do all that she did.

Alas, Polly has decided to hang up her weekly editorial role (but will happily remain our trusted Senior advisor). We wish her much joy as she looks ahead to other adventures and, hopefully, some relaxation. But she will always be the founding editor and honorary publisher, because every Weekly NewsDigest will reflect her legacy and continue to curate stories from countless sources covering the world of HIV prevention research — and we know who our closest reader will be each week!

Polly’s wisdom, insight, humor, passion and commitment are all models to us at AVAC – and, undoubtedly, to the field at large. While we can imagine (barely) the Digest without her, we can’t imagine this field without her influence!

On Thursday, October 26, colleagues from the Treatment Action Group (TAG), along with advocates, researchers and GPP practitioners, discussed the recommendations from the TAG’s recently released white paper, HIV Research in the Era of PrEP: The Implications of TDF/FTC for Biomedical Prevention Trials.

The effectiveness and increasing availability of oral PrEP raises questions about how it should be incorporated into clinical trial designs for next-generation HIV prevention options. This paper explores ethics, the scientific literature to date, and advocates’ views on the issue.

DAIDS Boss Muses on the Future of HIV Prevention Research

“You [advocates] push us as hard as you can. It’s your job, and you’re always mad,” said Carl Dieffenbach, Director of NIH’s Division of AIDS (DAIDS), who oversees a global HIV/AIDS research portfolio of more than US$1 billion. “And we’re always failing but in the end, there’s success. Keep pushing.”

Success is no joke. A few of us advocates gathered with Dr. Dieffenbach in a hotel lobby in Paris, ahead of IAS 2017 in July, to discuss the future of HIV prevention research. We all acknowledged, almost giddily, the “Golden Age of HIV Prevention” we currently inhabit: two vaccine efficacy trials—one enrolling, the other about to begin; two antibody trials; injectable PrEP moving into efficacy studies; the potential addition of the vaginal ring alongside oral pills for PrEP; and, of course, the large-scale rollout of VMMC and treatment, which are starting to bring down HIV incidence in some places.

But there’s still plenty to get “mad” about. As it does every seven years, DAIDS is reviewing the structures it has in place for HIV clinical trial research networks focused on different candidate products. It’s evolved several times over the past two decades. The structure as it is now includes the following prevention-focused networks—HVTN, HPTN, MTN and IMPAACT—along with the ACTG. The current funding for these networks has three more years to run, but the network recompetition process is already underway to determine the focus and structure for HIV research from 2021 through 2027.

Below are some of Dr. Dieffenbach’s rather intentionally provocative ideas, which will for sure get varying degrees of attention, praise and criticism. But, like the DAIDS boss says, he likes it. Indeed, he wants the community to weigh in. It’s not merely for the sake of box ticking that DAIDS is soliciting feedback, he said; there’s a stated desire to get a sense of the community’s unbiased aspirations—as opposed to opinions of those with a horse in the race or their own research agenda.

But, as Dr. Dieffenbach reminded us, decisions will get made through the filter of science. He can’t change the laws of physics to bring miracle products through the research pipeline. Here are some of his musings about specific research candidates—challenges and promises, and the questions they may raise for the next clinical trial research network structure.

Whither MTN?
Vaginal rings and things

Over the past 10 years, the Microbicide Trials Network (MTN), along with the International Partnership for Microbicides has helped provide information on how women use vaginal rings and gels, as well as oral PrEP, and how these products work when used consistently or irregularly. IPM has submitted an application for regulatory approval of the dapivirine vaginal ring while, as Dr. Dieffenbach noted, vaginal tenofovir gel has been shelved: “Gels have failed. They were given the college try to the tune of US$1 billion. Women have run away from gels. The social behavioral side failed,” he said. “We need to listen to community.”

The DAIDS leader also pointed out the emerging data showing a higher rate of rectal sex in women than originally perceived, which could confound any vaginal protection offered by the ring. This is a challenge for any research network, regardless of future structural network configurations.

In the meantime, according to Dr. Dieffenbach, the licensing of the vaginal ring will be a good incentive for industry to take on next-generation products. For example, “The NuvaRing for birth control is off patent soon. Can it add ARVs?” he asked. “The ring studies are done; NIH will not go further but companies can,” he said. Rectal microbicides
The MTN 033 study will show how well a rectal dapivirine gel permeates the colon—comparing rectal administration via vaginal applicator and a coital simulation device (dildo). The tricky part is getting the right balance of volume and concentration, according to Dr. Dieffenbach. He warns a smaller volume may be more acceptable, but this would require higher drug concentrations, which could lead to irritation and higher risk. Likewise, he “worries about a suppository, which could burn the crap out of the rectum.” His concerns belittle a robust rectal microbicide research pipeline, boasting seven experimental drugs, four formulations—including douches and inserts—and eight different studies. However, the outstanding scientific questions, according to Dr. Dieffenbach, “do not need an entire network devoted to it.”

Such a stirring statement raises several questions: If there is no MTN, what will happen to the pipeline, the expertise and the hundreds of millions of dollars spent? Will DAIDS not pursue its first phase III trial even after the phase II MTN 017 study results in 2016 showed the gel to be safe and well tolerated? In the era of oral PrEP, do people not need an alternative, on-demand, non-systemic, non-committal way to protect themselves?

Injectables and implants
The HIV Prevention Trials Network (HPTN) has two major trials of the injectable ARV cabotegravir, one underway and the other close to launch. Given the buzz around less-adherence-dependent methods, it’s almost certain the next trials network will have infrastructure for testing other long-acting systemic PrEP methods.

These networks won’t be able to evaluate how an injection would be delivered in the real world though. “[Delivering] injections will require a change in infrastructure, away from pills in the clinic—for both treatment and prevention,” said Dr. Dieffenbach. “But NIH is terrible at implementation research,” he continued. “We need to partner with health departments.” Dr. Dieffenbach also anticipates future research into removable implants that might last 6–12 months.

DAIDS: Coming attractions
Results from an array of trials will shape DAIDS’ next steps. These include: HPTN 083 and 084, HVTN 703/HPTN 081, HVTN 704/HPTN 085, HVTN 702 and 705; and MTN 034/IPM 045. At minimum, the networks would have to be able to follow up on these vaccine, antibody-mediated prevention, long-acting PrEP and ring trials.

Dr. Dieffenbach suggested we look at the three-year time horizon when the current funding period will end. By then, we’ll have results from efficacy trials in the most at-risk populations including young women in eastern and southern Africa as well as young black gay men in the US. “We’ll have completed the vaccine trials and the VRCO1 [antibody-mediated prevention] study,” he said. “We’ll need to look at how to mix a semi-working vaccine with combination prevention in 2021–2027. Where there are successes, there’s a need to discontinue,” he explained. All this to say that in the next round of funding, DAIDS is looking at collapsing the current networks into just three—focusing on treatment, non-vaccine prevention and vaccines, explained Dr. Dieffenbach in a recent blog and AVAC webinar.

About PxROAR Transgender Africa

PxROAR Transgender Africa will offer HIV prevention education and advocacy assistance with the goal of creating a regional agenda for HIV prevention research and implementation for the transgender community. The program is modeled on AVAC’s PxROAR Africa Program, which is tailored for key populations. Advocacy for self-determination and free expression is mutually dependent on the goal to seek control and, eventually, end HIV epidemics in underserved communities.

PxROAR Transgender Africa has two primary goals

1) To enhance awareness of and advocacy for biomedical HIV prevention.
2) To contribute to a transgender presence at country and regional level articulating a rights and biomedical prevention agenda.

Time commitments

The PxROAR Transgender Africa program is voluntary and intended to complement ongoing work in HIV, gender and/or human rights advocacy. The time commitment depends on your own level of availability. Outside of a required monthly group call, you determine your level of engagement. However, AVAC is interested in partnering with those who maximize the opportunity by tapping into all the program’s offerings. Technical and financial support is commensurate with level of commitment.

Who should apply?

  • Transgender and non-conforming African advocates from Eastern and Southern Africa interested in expanding their knowledge of biomedical HIV prevention research and implementation, creating a roadmap for transgender and HIV prevention research, translating scientific information and mobilizing communities toward the ultimate goal of controlling HIV in highly burdened, underserved populations.
  • Ideal candidates will have the potential to integrate their PxROAR advocacy, complementing and benefiting their current work.

HIV Cure Research Strategy for Women: Where are we?

This blog post first appeared on Positively Aware. It represents a report taken from a session presentation summary at this year’s US Conference on AIDS (USCA), a community-based event which was held in September in Washington, DC.

The need for a cure is critical, but will the search for HIV cure strategies include women? Women are drastically underrepresented in HIV cure research studies. A review of 159 studies showed that only 18 percent of HIV cure study participants were women. Women have a high willingness to participate in research, although they are in general less willing than men to take risks related to HIV cure research interventions. Research is in the works to better understand the differences in willingness, but studies outside of the HIV cure arena suggest that if structural barriers to participation are diminished, and more effort is made to establish trusting relationships between investigators and participants, more women will participate.

Currently, there is no cure for HIV. Only one person has been cured, Timothy Ray Brown, after receiving two bone marrow transplants that simultaneously rid his body of HIV infected cells and gave him new cells that are resistant to HIV. Scientists are trying to replicate his cure. The transplant of stem cells is a central HIV cure strategy being investigated, particularly to eliminate nearly all traces of the virus from a person’s body (e.g., an eradicating, or sterilizing, cure). Other methods include the early administration of antiretroviral treatment, combined with a variety of strategies that make HIV more visible to infection-fighting immune cells, improving the immune system’s ability to detect and eliminate infected cells, and possibly rendering immune cells impervious to HIV infection.

Scientists suspect that there are gender-based differences that affect how these investigational interventions might work inside the body. For example, two studies have suggested that the presence of female hormones and hormone receptors on immune cells might make it more difficult to flush HIV out of hiding. In addition, cis-gender women who have female chromosomes are genetically primed for stronger immune responses to infection and to vaccines. They might, perhaps, have an advantage if vaccines or other approaches to prime the immune system’s response to HIV are used.

While studies of all diseases have traditionally lagged far behind in terms of participation by women, HIV has more recently had a better track record in terms of prevention and treatment trials, and improvements in recruitment and retention guided in part by the efforts of the Women’s Interagency HIV Study (WIHS), which is an NIH-funded cohort (or group) that studies the impact and progression of HIV infection among women in the United States. It is the largest and longest running HIV cohort focused on women. In total, 4,982 women at nine sites have participated in the cohort to date. The WIHS cohort is also uniquely diverse in that it is reflective of the US epidemic among women. The WIHS cohort is a great platform to advance HIV cure-related research in the United States. WIHS participants volunteer in studies that measure the HIV reservoir in the blood, genital tract, and other tissues, that study the role of sex hormones on HIV reservoirs and that examine the relationship with ART pharmacology.

The WIHS cohort has been creative in overcoming challenges to women’s participation in HIV cure research. For example, if women are asked to come in fasting for a lab test, the study site provides a substantive meal after the procedure. Providing mileage reimbursements, metro vouchers, and taxi rides are critically important, since transportation issues are a main deterrent to participation in research. Other strategies to improve sex equity in HIV cure-related research include addressing eligibility criteria, adapting recruitment strategies, and engaging community members as early as possible in the process. For this reason, WIHS could serve not only as a model for successful recruitment and retention of women living with HIV into research, but also as a source of participants who might be inspired to participate in HIV cure-oriented research.

Including women in HIV cure research means:

  • Thinking about what they need: different types of recruitment strategies and study coordination which may benefit from partnerships with different organizations or advocacy groups
  • Listening to what they want: information and a chance to participate, fair reimbursement and compensation, assistance with logistical barriers
  • Designing studies that reflect them: re-thinking eligibility criteria to balance participant risk with exclusions that disproportionally affect women
  • Prioritizing their involvement: a few women or a single study is not enough, we need to advocate for representation of all women, including transwomen
